Assisted conception treatment in the UK is administered by the Human Fertilisation and Embryology Authority (HFEA). The HFEA require that you consider all of the implications and consequences of undergoing fertility treatment and have developed a comprehensive set of consent forms. you will be given access to these forms through the clinic portal once you are rigistered. Rebecca, our fertility coordinator, will go through these with you and make sure you understand them before signing.
Many aspects of fertility treatment are intensely personal and communication regarding treatment details is treated with the utmost confidentiality. To ensure that you have complete control over who, amongst your medical care givers receives this information and to protect your privacy we will ask you to complete the HFEA Consent to Disclosure (CD) form.
The HFEA also require us to consider the welfare of any child born as a result of fertility treatment. We form in order to discharge this duty we need you to complete the HFEA Welfare of the child (WOC) form
